Description
ESAS-r is a validated patient-reported outcome measure that assesses the severity of nine common symptoms experienced by patients with serious or chronic illness: pain, tiredness, drowsiness, nausea, lack of appetite, shortness of breath, depression, anxiety, and overall wellbeing. Each symptom is rated by the patient on an 11-point numeric scale ranging from 0 (no symptom) to 10 (worst possible severity). An optional free-text field allows patients to record an additional symptom or concern not otherwise listed. The revised version includes clearer wording and definitions to improve patient understanding and data consistency.
