Description
NEWS2 assigns weighted scores to six physiological parameters: respiratory rate, oxygen saturation, need for supplemental oxygen, temperature, systolic blood pressure, heart rate, and level of consciousness (including new confusion using the ACVPU scale). The total NEWS2 score stratifies risk of acute deterioration into low, medium, or high, prompting specific clinical responses and escalation pathways. This implementation follows the Royal College of Physicians (RCP) NEWS2 update, including dual oxygen-saturation scales and considerations for hypercapnic respiratory failure. The form is structured to enable consistent documentation of vital signs, automatic score calculation, and clear visibility of risk categories, supporting safe monitoring across diverse care environments.
