Description
The qSOFA Score evaluates three validated clinical criteria associated with increased mortality in patients with suspected infection: altered mental status (GCS < 15), systolic blood pressure ≤ 100 mmHg, and respiratory rate ≥ 22 breaths per minute. Each criterion contributes one point, producing a score from 0 to 3. A score ≥ 2 is associated with a higher likelihood of poor outcomes, prompting clinicians to consider further assessment, investigations, and interventions. The form captures each parameter in a structured format, provides automated scoring, and supports integration into sepsis screening workflows or early warning dashboards.
